We would love to see you as a patient for you to get the answers and relief you need and deserve.

We don’t take insurance at this time, but we can make you a ‘Superbill’ that you can self submit for partial or full reimbursement from your insurance company. You can also use FSA/HSA funds if applicable.

*Most medications prescribed are FDA approved and may be submitted through insurance for coverage based on your plan. Any lab requests will also be able to be submitted through insurance based on individual plan coverage. 

Cancellation: You must cancel your appointment at least 24 hours before the scheduled appointment to avoid a $50 cancellation fee.

Credit card required at time of booking is only charged if less than 24 notice of cancellation or after office visit is complete. May choose alternate form of payment at time of appointment.
